If you want to reset your password and you know your PIN then enter your pin in the box below and click the 'Reset Password' button.
You will receive an email with instructions on how to reset your password.
If you do not know your PIN then you must contact your survey practitioner or administrator and ask them to reset your password for you.
If you do not know who your practitioner or administrator is or how to contact them then enter one of the pieces of information below and we will try to locate that information for you.
After they submit one of the pieces of information we will search the database and display the results, either the practitioner or the user. It will list the practitioner(s) contact information so they can email or call the practitioner and request their PIN.
I don't think we should automate the PIN retrieval process, because some of the participants may be using the same email. But we could if we decide to because the PIN information will be sent to the email address for the registered participant so I don't think it will be a security problem. But this way the Practitioner (or Admin) can make sure they have the correct participant.
